Privacy statement summary
Tu Tonu Hauora will protect personal and health information we collect and will treat it with care, respect, and discretion. We will comply with the Privacy Act 2020 and any other obligations we have relating to personal information. If you have any concerns or questions you can contact the practice manager either by phone or website enquiry.
Privacy statement
Patients have the right to access, and have corrected, their health information under Rules 6 and 7 of the Health Information Privacy Code 1994.
Information provided on enrolment forms will be recorded and used by Tu Tonu Hauora to obtain a National Health Index (NHI) number OR update any changes OR determine eligibility to receive publicly-funded services. Information may be compared with other government agencies but only when permitted under the Privacy Act.
If a patient visits another doctor who is not their regular doctor, they will be asked for permission to share information from the visit with their regular doctor or medical centre. If a patient visits another doctor they can make a claim for a subsidy, and the medical centre the patient is enrolled with will be informed of the date of that visit. The medical centre visited and reason(s) for the visit will not be provided unless the patient provides consent.
The Tu Tonu Hauora health team may use and update the patients health record to provide appropriate care. Health information may be shared to other health professionals to assist in providing appropriate care. Health information may be sent to external health organisations managing programmes such as immunisation or breast screening. Health information, excluding patient name, may be sent to district health boards and the ministry of health for service planning and quality monitoring but health information will not be used or published in a way that will idenify the patient.
A financial auditor may review patient health information when checking financial claims made by Tu Tonu Hauora as set out in the terms and conditions of Section 22G of the Health Act or any subsequent applicable Act.
Except as listed above, information about a patients health status and services received will remain confidential within Tu Tonu Hauora unless specific consent is given by the patient.
